How Do I Update My Name or Address?

Address change – current city employees

You must change your address online through the city's eCHRIS human resources database. Demographic information from eCHRIS, including address changes, is transmitted daily by the city to Nationwide. If you make address changes, but your changes are not reflected on your account (please allow 24 hours), please contact the city's HR Department/Benefits Division at 602-256-3282.

Address change – former city employees

You can change your address online, over the phone, via fax or by sending a letter.

Change your address via mail or fax by sending us this information:

  • Name
  • Account number or the last 4 digits of your Social Security number
  • Address
  • Telephone number

Name change

You can change the name on your account via mail or fax by sending us:

  • Former name
  • Account number or the last 4 digits of your Social Security number
  • Address
  • Telephone number
  • A photocopy of a legal document that shows your name change, like a marriage certificate or driver's license
